This position is a community based clinician that will support integrated mental health care across inpatient and community sites. Based in one of the two Assertive Outreach Teams in SLHD, this position will work within community mental health services to support complex care coordination for long stay consumers and ensure community tenure, working across the two assertive outreach teams and long stay inpatient services. Central to this role will be working with inpatient team to effect transfer of care.
 
The Senior Clinician, Assertive Outreach position has four primary roles. Firstly, this position will provide direct clinical care. This will include community based complex care coordination and supporting discharge planning for long stay mental health consumers, working closely with inpatient units on effective transition plans and providing consultancy as required. Secondly, this position will support workforce skill development in inpatient and community teams enabling staff to better support people with complex mental health presentations and multiple comorbidities, ensuring best practice recovery oriented care. Thirdly, the position will have accountability for ensuring PCLI assessments and reporting are completed, as directed. Finally, this position will facilitate the development of a range of CMO packages and supports which ensure joint care planning, complement clinical care and facilitate community tenure.

Ideal Candidate

  • Tertiary qualification in Occupational Therapy or Social Work with full AHPRA registration (O/T) or eligibility for membership to AASW and experience in mental health. For CNC current nursing registration with AHPRA with a minimum of five (5) years full time equivalent post registration experience and three (3) years full time equivalent post registration experience in mental health. Approved post graduate qualifications relevant to the field or such other qualifications or experience deemed appropriate by the organisation.
  • Demonstrated effective written and verbal communication skills, time management and computing skills, and current NSW Drivers Licence (P1 and P2 acceptable).
