What you'll be doing
The Rehabilitation Case Manager (RCM) is part of the Rehab2Kids department and is responsible for co-ordinating, monitoring and evaluating the provision of care for eligible children and young people with complex physical and/or cognitive needs, as part of their overall rehabilitation management plan. The Care Manager will work in collaboration with clients and families and the Rehab2Kids multi-disciplinary team, to facilitate discharge planning and navigate the numerous pathways required to manage a child or young person in their community. This involves liaison with stakeholders in community agencies, other government agencies, private providers and educational facilities to ensure an integrated service for the clients rehabilitation needs
  1. Tertiary qualifications and registration in accordance with NSW Health Service Health Professional (State) Award.
  2. Extensive clinical experience in paediatric rehabilitation of children and young people including following an acquired brain injury
  3. Approved iCare/LTCS Case Manager status or willingness to work towards same, with high level and current working knowledge of NSW CTP, Workcover and iCare/LTCS
  4. Demonstrated ability to communicate effectively with clients, families, health professionals, and government agencies (i.e. the National Disability Insurance Agency) including high level negotiation and conflict resolution skills
  5. Demonstrate the ability to work independently as well as consult and collaborate with an interdisciplinary team
  6. Current NSW drivers licence
  7. Demonstrated experience in effectively developing rehabilitation plans and independently case managing clients
  8. Competent use of computer applications and knowledge of electronic patient records and systems
