About Us

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Are you a Registered Nurse looking for your next career move? We have on offer an exciting opportunity, working permanent full time Monday - Friday for an experienced Registered Nurse to provide senior clinical leadership in the development of specialist clinical practice and service delivery within a nursing unit, team or service. The successful CNS2 incumbent will employ advanced clinical skills and exercise extended autonomy of decision-making working closely with managers and other senior clinician to provide safe, compassionate, person-centred health care in collaboration with patients, families, carers and multidisciplinary teams. 

The role will see you work within a primary health care framework and using a client centred and strengths-based approach. Within this role you will provide assessment, planning, implementation, and evaluation of care using evidence based best practice guidelines underpinned by the principles of Primary Health Care as part of a collaborative partnership with the client, their family(ies)/carer(s) and other key stakeholders (including the client's GP and treating specialist).

You will be a member of the Community Health Care Secondary Support Nursing Wound Care Team and will 
work with other community health teams – allied health and specialty programs, general practitioners, hospital teams and specialist wound care teams to co-ordinate and deliver evidence based best practice wound care to clients with acute and chronic wounds. The goal of the Community Health Care team is to promote and maintain a client's function and independence at home, and to provide support for their family and carers.

What you will bring to the role

  1. Current registration as a Registered Nurse/Midwife Division 1 with AHPRA.
  2. Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.
  3. Demonstrated capacity to provide effective nursing/midwifery care that is person centred, family inclusive and culturally sensitive to people accessing health services.
  4. Demonstrated ability to perform in a clinical leadership role and use critical thinking as it relates to the delivery of advanced evidenced based nursing care.
  5. Demonstrated effective communications written and verbal skills.
  6. Demonstrated commitment to professional development.
  7. Ability to work independently with minimal supervision maintaining involvement within the wider multidisciplinary team
  8. Current and valid NSW Drivers Licence and willingness to use for work purpose travel (this may include driving long distances across the LHD)
