About Your Role

The Carer Network Coordinator – Integrated Care will function as a member of the MLHD Integrated Care Team.

This position will develop and lead the coordination of Community Carer Services, ensuring that the planning, organisation and scheduling of carer services supports MLHD organisational goals.

This role will enhance health education/promotion and service capacity building projects and initiatives. 

They will provide high level advocacy around carers with a particular focus on empowering carers to provide care for their loved ones within the community and building of a carer support network.

The Carer Network Coordinator will facilitate development of partnerships with carers, consumers, clinicians, managers, government and non-government agencies as well as community organisations and support groups.

This position is open to the following classifications, as per Position Description: CNS/CMS - Grade 2, Dietitian - Level 3, Occupational Therapist - Level 3, Physiotherapist - Level 3, Social Worker - Level 3, Speech Pathologist - Level 3.
