Remuneration: $135,826.04 - $154,025.92 pa (+ superannuation + salary packaging options)

Nurse Practitioner/ Transitional Nurse Practitioner – Aged Care Outreach Service

About the role
An exciting opportunity exists to become part of the team to develop and implement a new model of care in a rural context.

Western NSW Local Health District (LHD) is establishing an Aged Care Outreach service for older people with acute illness who are at risk of hospitalisation. Western NSW LHD provides care to people living in an area covering about 1/3 of NSW.

The service will have a network of staff based at multiple locations across Western NSW and will provide a hybrid face to face and telehealth outreach service.  You will join a team which includes geriatricians, registered nurses, a pharmacist, a nurse manager and admin staff and will work closely with the Aged Care team, clinical staff in our hospitals and community settings as well as staff in Residential Aged Care Homes.

You work will be a mix of outreach into residential aged care, outpatient clinics and acute care consultations. The service will initially operate 8-5pm Monday to Friday with the option to extend hours as the service develops.
