Provide clinical pharmacy services supporting Hospital in the Home patients across inpatient and community settings.

Remuneration: $42.23 - $62.50 per hour
Hours Per Week: 16 (Job Share may be considered for part time hours)
Location: Cumberland Hospital - HITH Cumberland East Campus
Where you'll be working
The HITH service provides care to patients in hospital, outpatient and community settings across WSLHD.
Based at Cumberland Hospital East Campus
Works within multidisciplinary HITH team
Reports to Pharmacy Managers and Clinical Leads
Interfaces with community pharmacists and clinicians
Provides services in patient homes and health facilities
What you'll be doing
You will provide pharmaceutical services across the HITH program.
Manage medication distribution and ensure safe supply of medicines
Provide clinical pharmacy services and medication advice
Conduct home visits for reviews, education and medication delivery
Prepare and supervise sterile and non-sterile products
Participate in quality improvement and service delivery programs

Essential Criteria:
· Qualification: Pharmacy qualifications conferred by Tertiary Education Institutions e.g Bachelor of Pharmacy
· Registration: Full and unrestricted Pharmacist Registration with AHPRA
Selection Criteria:
1. Demonstrated excellent communication skills
2. Demonstrated excellent therapeutic knowledge.
3. Demonstrated ability to prioritise workload and have good time management skills
4. Demonstrated ability to work independently as well as in a team environment
5. Recent hospital work experience or community pharmacist position with accreditation.
