Clinical Nurse Specialist Grade 2, Acute to Aged Related Care Services - Bulli Hospital

1. Current Registration with the Nursing & Midwifery Board of Australia (Division 1).
2. Relevant post registration qualifications in the field of Aged Care and at least 3 years’ experience in Aged Care nursing.
3. Demonstrated ability to work both as part of a multidisciplinary team and as an independent practitioner with a high degree of autonomy.
4. Excellent written and verbal communication skills and interpersonal skills.
5. Demonstrated experience and knowledge of case management, including assessment and care coordination for older people with complex needs.
6. Extensive knowledge of community-based and other non-hospital support services for aged care including their referral pathways and functional scope.
7. Understanding of the role and impact of the Commonwealth Aged Care Act (1997), the My Aged Care Program and Aged Care Assessment Teams on aged care services.
8. The ability to act as a resource person for hospital staff including the ability to provide expert advice on best-practise standards for aged care.
