As a Care Navigation Clinical Nurse Specialist, you’ll be part of a committed Child and Adolescent Mental Health team working to ensure children, adolescents and their families receive timely, safe and coordinated access to care. 

In this position, you will apply specialist child and adolescent mental health knowledge to triage and assess referrals, manage referral pathways and coordinate access to appropriate services for young people aged 5–17 and their families. You will also undertake risk screening, develop safety plans, facilitate systems coordination and provide single session support when required. 

Your work will extend across Nepean and Blue Mountains Hospitals, with the possibility of supporting services in Lithgow and Hawkesbury. As a consultation liaison clinician, you’ll provide guidance for children and adolescents presenting to or admitted within hospital settings. Collaboration is at the heart of this role, and you’ll work closely with an interdisciplinary team and a range of internal and external providers to support safe, coordinated and effective care.

What you will bring to the role

  1. Current registration as a Registered Nurse/Midwife Division 1 with AHPRA.
  2. Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate in child and adolescent mental health.
  3. Demonstrated capacity to provide effective Nursing/Midwifery care that is person centered, family inclusive and culturally sensitive to people accessing health services.
  4. Demonstrated ability to perform in a clinical leadership role and use critical thinking as it relates to the delivery of advanced evidenced based nursing care.
  5. Demonstrated effective communications written and verbal skills.
  6. Demonstrated commitment to professional development.
  7. Ability to work independently with minimal supervision maintaining involvement within the wider multidisciplinary team
  8. Ability to work a flexible roster, day evening or night shift across all days of the week according to the unit or team requirements
