About the Role
As a Clinical Nurse Consultant (CNC) in the Integrated Care for Aboriginal People Program (ICCAPP), you’ll lead the way in delivering culturally appropriate, person-centred care for Aboriginal people aged 15 years and older living with—or at risk of—chronic diseases such as cancer, diabetes, cardiac, respiratory, renal disease or stroke.
Working in close partnership with a dedicated Aboriginal Health Practitioner, your role spans the Mehi area, with regular travel to community health sites. You will:
- Collaborate with clients, families and local services to deliver early intervention, secondary prevention and self-management programs.
- Support Aboriginal Community Controlled Health Services and other primary care providers through assessment, case reviews and development of clinical management plans.
- Promote best practice, build local capacity, and advocate for culturally safe care through strong leadership and consultancy.
About You
You're an experienced clinician—likely from a larger site—with strong expertise in chronic care or diabetes, and a genuine commitment to Aboriginal health.
