The Role

The Community Transitions Occupational Therapist will be part of a multidisciplinary team and works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health Services and other government and non-government organisations (NGOs), in a clinical and consultative capacity to ensure continuity of care. By understanding patients’ clinical, psychosocial and behavioural vulnerabilities this role will utilise general case management and discipline specific skills to link patients to relevant services in the community and support their initial reintegration to community care.

The Community Transitions Team is an arm of the Statewide Community Forensic Mental Health Service.
