Position Title: Connections Transition Coordinator

 This position works from our Sydney Olympic Park Office and regularly attends Sydney Metro Based Correctional Centres as a part of regular work duties.

The Role

The Connections Program aims to improve continuity of care for patients who are in public correctional Centre with Drug and Alcohol problems, that are being released to the community. The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers pre and post release.
 
The Connections Transition Coordinator will utilise a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.
