The Hospital in the Home program provides hospital-level care in patients' homes, supporting acute wards and alleviating pressure on hospital beds. Our team, including nurses and doctors, manage both home-based treatment and the infusion ward, which handles IV therapies and transfusions. This integrated approach ensures seamless care across hospital and home settings, enhancing patient outcomes and efficiency.

In this role, you will provide safe, compassionate, person-centred care with a high level of autonomy to patients within the Maitland region and surrounding areas. You'll have the opportunity to work independently while collaborating with multidisciplinary teams (MDT), ensuring that patient needs are met in the home setting. This position will be on a 7-day rotating roster, however, is day shift only. 


Key responsibilities:
  • Provide nursing care, including IV medications, drain care, and phone consultations with patients
  • Independently manage a patient list, visiting patients in their homes and providing on-the-spot clinical care
  • Work collaboratively with doctors and liaise with the MDT to ensure optimal patient outcomes
  • Advocate for patients and their families, offering guidance and support throughout the care process
  • Maintain accurate patient documentation and care plans
  • Use your strong clinical background to problem-solve and address complex patient needs in a home setting


About you:
The ideal candidate will be a compassionate, proactive professional with a strong clinical background. You must be self-reliant, adaptable, and able to work independently while remaining an integral part of the team. You’ll have excellent communication skills to liaise effectively with doctors, families, and the MDT. Strong advocacy skills are essential, as well as the ability to remain calm and focused in challenging situations.


Essential criteria:

  • Current registration as a Registered Nurse with AHPRA
  • Eligible to drive in NSW
