The Program You Will Support

Join a passionate, multidisciplinary team delivering the Sustained Nurse-Family Partnership (SNF) Program, a home visiting service supporting vulnerable families and children to achieve positive long-term outcomes. 

This role will operate out of our Wallsend Service offering meaningful, community-based nursing practice. We are also looking to create a strong eligibility list for our services across our Raymond Terrace/Port Stephens through to the Lower Mid North Coast areas!  


We currently have two key roles, both focused on building strong, therapeutic relationships with families through regular home visiting and ongoing support.

  • SNF Program – a coordinated, nurse-led model of care
  • SNF PLUS Pilot – a collaborative model of care delivered jointly with Social Work
     



What You Will Contribute

In this role, you will work in partnership with families to identify goals and support positive outcomes, always keeping the baby at the centre of care. 

You’ll provide sustained follow-up through home visiting and contribute to collaborative, multidisciplinary case discussions.


Your key responsibilities:

  • Conducting regular home visits to support families and children

  • Working in partnership with families to set and achieve strengths-based goals

  • Participating in weekly multidisciplinary case conferences

  • Providing therapeutic, relationship-based nursing care within professional boundaries

  • Contributing to coordinated care planning with nurses, social workers, and allied health

  • Strong ability to build therapeutic, professional relationships

  • Experience working respectfully in partnership with families and clinicians

  • A compassionate, non-judgemental and child-focused approach

  • Strong communication and reflective practice skills

  • Ability to maintain appropriate professional boundaries

  • Essential: Hold postgraduate qualifications in Child and Family Health Nursing & willingness to complete Advanced Nurse Practice course within 12 months of employment.
