About the location:
Located in the picturesque Upper Hunter region—renowned for its stunning wilderness areas, distinctive wineries, world-class horse studs, historic homesteads, and warm country hospitality—this Physiotherapy position is an exciting opportunity within the Hunter Valley Community Health Transitional Aged Care (TACP) team at Muswellbrook Hospital. As the sole TACP Physiotherapist based at Muswellbrook, you will play a vital role in the multidisciplinary team on site, while also being part of a broader network of TACP Physiotherapists located at Singleton and Kurri Kurri, fostering collaboration and shared expertise across the region.

About the role:
As a Level 3 Physiotherapist, you will be responsible for delivering physiotherapy services to community clients enrolled in the Transitional Aged Care Program (TACP). Your role will involve providing client-centred, goal-based physiotherapy within a community setting to eligible patients. A typical week will see you managing a diverse caseload that includes orthopaedic conditions, falls prevention, geriatric neurological issues, respiratory care, and patients recently discharged from long hospital stays. You will collaborate closely with patients and their families to develop realistic, goal-focused treatment plans aimed at promoting independence and improving quality of life.

Although you will be the sole physiotherapist on site, you will play a vital role within the Muswellbrook TACP multidisciplinary team (MDT), working alongside local health professionals to ensure coordinated and comprehensive care. Additionally, you will have access to ongoing clinical support from physiotherapy teams based in Kurri Kurri and Singleton, as well as the Physiotherapy Head of Discipline, who is also located at Muswellbrook.

About you:
In this role, you will provide comprehensive physiotherapy assessments and interventions tailored specifically for aged care clients in a community setting. You will develop realistic, goal-oriented treatment plans designed to promote patient independence and enhance quality of life. Collaboration is key, as you will work closely with patients, their families, and multidisciplinary team members to ensure coordinated and effective care. Managing a diverse range of complex patient presentations, you will apply strong clinical reasoning and therapeutic skills to deliver the best outcomes. Additionally, you will engage in ongoing professional development, supported by experienced clinical mentors and colleagues to continually refine your practice.
We are seeking a motivated and adaptable physiotherapist who demonstrates:

  • Strong clinical reasoning and assessment skills in aged care and rehabilitation
  • Excellent communication and interpersonal skills to engage with patients, families, and multidisciplinary team members
  • Ability to work independently while knowing when to seek support or collaborate
  • Empathy and a person-centred approach to care delivery

     


Previous experience in transitional care or community physiotherapy is highly desirable but not essential — a willingness to learn and develop these skills will be supported.

Requirements:

  • Physiotherapist with current Authority to Practice with AHPRA
  • Eligibility to drive in NSW
