Remuneration: $40.05 to $63.29 per hour + superannuation + salary packaging
Hours Per Week: 24 hours per week

About the role:
As a Health Case Manager, you'll work closely with children and young people, their carers, and caseworkers to deliver holistic, trauma-informed health assessments and care planning. You'll identify health needs, coordinate care, and connect clients with the services and supports they need to achieve better health outcomes.
You'll also provide health education and advocate for children and young people to ensure their physical, emotional, and developmental health needs are met. Working across the Local Health District, you'll build strong partnerships with health, community, and government services, making a meaningful difference in the lives of children, young people, and their families.

Our team provides a collaborative and supportive environment where you'll be backed by experienced Health Case Managers, a Clinical Lead, and the District Coordinator.

About you
We're looking for someone who is passionate about improving the health and wellbeing of children and young people. You'll bring an understanding of culture and community and be committed to providing culturally safe, trauma-informed care.
You will have experience working with children and young people to assess and respond to their health needs, along with the ability to build positive relationships with clients, families, carers, caseworkers, and partner services. You'll be a proactive and adaptable team member who enjoys working both independently and collaboratively, with strong communication skills and a commitment to delivering high-quality, person-centred care.
Requirements
Must hold a degree in Social Work, Nursing, Occupational Therapy, Physiotherapy, Psychology or Speech Pathology or other qualification deemed equivalent by the employer, which provides eligibility or membership of the relevant professional association/full registration through the Australian Health Practitioner Regulation Agency.
Minimum of 3 years experience relevant to working with children and young people with a thorough understanding of trauma informed and culturally safe practice. Demonstrated understanding of culturally competent healthcare and effects of generational trauma
