Nurse Practitioner / Transitional Nurse Practitioner – Diabetes Service
You will be working alongside a team that provides inpatient and outpatient diabetes care for people of all ages with Type 1 and Type 2 diabetes, and diabetes in pregnancy.

We’re looking for a passionate Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) to provide expert diabetes care in a fast-paced, community-focused environment where your leadership, clinical expertise, and compassion will truly shine.
Where You’ll Be Working
Based at the Tamworth Community Health Service, located within Tamworth Hospital, you’ll work alongside a small, dedicated team of Registered Nurses and a Dietitian. Together, you’ll deliver comprehensive, patient-centred diabetes care across both acute and community settings.
What You’ll Be Doing
As a Nurse Practitioner in Diabetes, you’ll deliver advanced clinical care using your highly developed nursing knowledge and skills.
Your responsibilities will include:
Conducting comprehensive assessments and managing patients with diabetes
Initiating and interpreting diagnostic investigations
Prescribing medications within your scope of practice
Referring patients directly to other healthcare professionals as required
Enhancing the interface between acute and primary care services through collaboration and communication with multidisciplinary teams
You’ll provide care to people of all ages across acute, outpatient, and community settings, making a real difference in diabetes management and prevention

Requirements
Current endorsement as a Diabetes Nurse Practitioner, or working toward endorsement (Transitional Nurse Practitioners welcome)
Current AHPRA registration as a Registered Nurse
Minimum 3 years full-time advanced practice experience in diabetes (within the past 6 years)
Postgraduate qualifications at master’s level (or currently completing) in an NMBA-approved program
