About the Role

This is a high-impact leadership opportunity for an experienced nurse leader who thrives on problem-solving, collaboration and system improvement. On any given day, the Discharge Liaison Manager is resposible for providing operational and clinical leadership for hospital-wide discharge planning processes; monitoring patient flow, length of stay and discharge barriers, implementing strategies to improve efficiency and patient outcomes; and acting as a key liaison with internal and external stakeholders to facilitate continuity of care post-discharge. You will use data and reporting to identify trends, risks and improvement opportunities, and in turn contribute to quality improvement initiatives, audits and service redesign. If you're a proactive leader who enjoys driving change and seeing measurable results, look no further than Tamworth Hospital.

You won't be working alone - you will be supported by the Deputy Director of Nursing and Midwifery (DDONM) and Director of Nursing & Midwifery (DONM), as well as your fellow Nurse Unit Managers (NUM's). You will have access to leadership development programs, mentoring, coaching and ongoing professional development, including study leave opportunities. This senior nursing leadership role offers genuine influence on hospital performance and the opportunity to shape and improve discharge and patient flow systems.
