Where you'll be working:
The NSW Health Aged Care Assessment Service (Hunter) is a large and supportive team with main office located at Wallsend, we have multiple team sites covering the Hunter including Newcastle, Lake Macquarie, Port Stephens and Lower Hunter.

The service provides quality assessment services to older persons in community and hospital settings, including 26 public and private hospitals, and 72 residential aged care facilities in above LGAs.

About the role:
Join our dynamic team as a Comprehensive Assessor and play a key role in supporting older people to access the care they need, when they need it.

In this rewarding role, you’ll work closely with older individuals and their carers to understand their unique goals, needs and preferences, and develop tailored, person-centred support plans. You’ll be part of a collaborative multidisciplinary team - working alongside Geriatricians, GPs, Practice Nurses, health professionals and non-government service providers - to ensure holistic, high-quality care.

This varied role offers the opportunity to work across a range of settings, including public and private hospitals, community services, and residential aged care facilities (RACFs), keeping your work meaningful, flexible and engaging.

The successful applicant will be well-supported from day one, with a comprehensive orientation and education program, including access to the National Training Program. This initial training period will span approximately two months and be based at Wallsend, providing a solid foundation for your role. Following the training and probation period, you’ll be allocated a primary work location based on service needs and team capacity. With service sites across the region - including Wallsend, Rankin Park, Newcastle, Toronto, Windale, Nelson Bay, Raymond Terrace, Kurri Kurri, and Maitland - you’ll be part of a well-connected team with ongoing support and opportunities to contribute where you’re most needed.

About you:
We’re seeking a motivated and experienced clinician who is passionate about supporting older people and ready to take on a rewarding role in the evolving aged care assessment space. You’ll have a solid understanding of the aged care system, backed by at least two years of hands-on experience in aged care, and be confident in navigating the complexities of client needs, services, and support planning.

To thrive in this role, you’ll need excellent communication and problem-solving skills, the ability to manage your time effectively, and the flexibility to work both independently and as part of a multidisciplinary team. You’ll also bring a high level of computer literacy, a current NSW driver’s licence, and a willingness to travel for assessments and training. Requirements:

  • Must hold a qualification in one of the following: Occupational Therapy, Nursing, Social Work, which provides eligibility or membership of the relevant professional association / provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA).
  • Eligibility to drive in NSW including willingness to travel to conduct home visit assessments.
