Where you’ll be working

Tamworth Hospital, within Hunter New England Local Health District (HNELHD), is at the heart of care delivery for Aboriginal communities in the region. HNELHD is one of the largest and most diverse health services in NSW, with a commitment to Closing the Gap and delivering culturally safe, patient-centred care.

These roles provide a unique opportunity for candidates with a strong cultural connection to the Tamworth region and the surrounding district and knowledge of local Aboriginal health services to directly influence the patient journey.
 

What you'll be doing
 As an Aboriginal Cancer Care Coordinator, you will play a pivotal role in supporting Aboriginal patients and their families throughout their cancer care journey. You’ll ensure services are culturally safe, accessible, and responsive, while building strong connections between patients, health services, and the community.

These positions will:

  • Work to support Aboriginal patients and families to navigate cancer services with dignity and respect.

  • Provide leadership in care coordination across clinical units, services, and community providers.

  • Advocate for improved patient experiences by embedding culturally safe practices.

  • Build partnerships with community organisations, Aboriginal healthcare workforce, clinicians, and service leaders.

  • Facilitate communication between patients, families, and healthcare teams to improve health outcomes.

  • Support transition, planning, and evaluation of programs that enhance service delivery for Aboriginal People.

  • By improving coordination and access, you will help ensure Aboriginal patients can focus on healing while the system supports their care journey.

About you

To be successful, you will have:

  • A cultural connection to Tamworth and the surrounding Aboriginal community.

  • Knowledge of Aboriginal health services and the barriers Aboriginal People may face when accessing care.

  • A passion for improving Aboriginal health outcomes and patient experiences.

  • Strong communication, relationship-building, and advocacy skills.

  • Relevant qualifications or equivalent experience in health, social science, or community service.

  • Experience in program planning, evaluation, or coordination will be highly regarded.
