This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
The Registered Nurse (RN) – Dementia service is responsible for providing specialist nursing support to patients experiencing cognitive impairment due to dementia diagnosis, including those presenting with Behavioural and Psychological Symptoms of Dementia (BPSD). The role operates across hospital and community settings, including Residential Aged Care Facilities (RACFs), with a strong focus on continuity of care and patient-centred outcomes.
You Will:
- Provide assessment, planning, and management of patients with dementia and BPSD within Central Coast hospitals, Community and RACS settings.
- Support the development and implementation of person-centred care strategies to address behavioural and psychological symptoms.
- Facilitate safe and effective transitions of care from hospital to community settings, including RACFs.
- Collaborate with patients, carers, and families to provide education, guidance, and support regarding dementia care.
- Work closely with multidisciplinary team members, including Registered Nurses, Clinical Nurse Specialists, the Clinical Nurse Consultant, and allied health professionals.
- Contribute to care coordination and ensure continuity of services across acute and community environments.
- Maintain accurate clinical documentation and adhere to relevant policies, standards, and guidelines.

About You
You will bring a strong commitment to high-quality community care and a passion for helping people maintain their health, independence, and dignity. You will have:
- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA). Current unrestricted NSW drivers licence.
- Demonstrated experience using advanced clinical skills in assessment and care planning for people with Behavioural and Psychological Symptoms of Dementia (BPSD).
- Recent clinical experience in BPSD and commitment to quality improvement, practice development and evidence-based practice.
- Demonstrated effective clinical communication through the use of information technology and sound written, verbal and interpersonal skills.
- Understanding of team nursing and demonstrated ability to work effectively and collaboratively as part of the multi-disciplinary health care team.
