As a Physiotherapist Level 1/2 within the Transitional Aged Care Program, you will play a vital role in the provision of Physiotherapy Services to the Transitional Aged Care Program (TACP) clients in its community (Transpac) and residential (Transitional Care Unit) services.

About the Role

This role focuses on collaborative care, delivering evidence-based interventions and working closely with other professionals, families, and carers to ensure our patients are supported holistically. Working within a multidisciplinary group of healthcare professionals, you’ll provide high-quality physiotherapy services that support clients in achieving optimal health outcomes.

You’ll be part of the Transitional Aged Care Program (TACP), which offers time-limited, goal-oriented rehabilitation to individuals transitioning from hospital care. Services are delivered through both community (Transpac) and residential (Transitional Care Unit) settings, targeting improvements in mobility, independence, and overall well-being. Your clinical expertise will help bridge the gap between acute care and long-term support.

About You

  • Current registration to practice as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Experience working in a multidisciplinary team environment, collaborating with nurses, occupational therapists, social workers, and medical staff

  • Understanding of aged care needs, including mobility, falls prevention, pain management, and functional recovery

  • Clinical assessment and treatment skills in physiotherapy, particularly with older adults or in rehabilitation settings.

  • Ability to work across sites within the CCLHD
