About the Role
In this role you will
Lead and inspire a dedicated team, fostering a culture of collaboration, excellence, and professional growth.
Ensure safe, patient-centred care for individuals requiring acute and post-acute services, including Hospital in the Home.
Work collaboratively with multidisciplinary teams to ensure smooth transitions and timely access to care across the healthcare journey.
Champion evidence-based practice, quality improvement, and service development initiatives to enhance patient outcomes.
Apply advanced clinical expertise to provide guidance, education, and support to staff, patients, families, and carers.

About You
- You’re a Registered Nurse (Division 1) with current AHPRA registration and relevant postgraduate qualifications.
- You bring at least 3 years’ clinical experience in acute, post-acute, or Hospital in the Home care and you're confident applying advanced nursing skills in complex situations.
- You’re passionate about providing safe, patient-centred care, especially for individuals transitioning from hospital to home.
- You demonstrate strong clinical leadership, mentoring and supporting other nurses while promoting professional growth.
- You hold a current NSW Driver’s Licence and are comfortable working in both hospital and community settings.
- You’re enthusiastic about continuous learning and development, both for yourself and your team.
