Social Worker Level 3 – Community Senior Clinician (End of Life)

About The Opportunity

The Senior Social Worker is a key member of a community-based multidisciplinary allied health team delivering person-centred services across the Central Coast. Community Allied Health supports clients and their families across a range of community settings, with this role providing specialist End of Life social work services within a dedicated interdisciplinary team environment.

This senior clinical role has significant responsibilities in the delivery, coordination, and monitoring of social work services for referred clients and their carers. The primary clinical focus includes comprehensive psychosocial assessment, counselling, case planning, advocacy, liaison, and referral to assist individuals and families in managing the emotional, social, and practical impacts associated with progressive illness, end-of-life care, loss, and bereavement.
The role requires close collaboration with interdisciplinary colleagues and senior managers to ensure the provision of responsive, evidence-based, and proactive social work services.

In addition to clinical responsibilities, the Senior Social Worker contributes to service development, quality improvement initiatives, program evaluation, and the education and support of staff and students. Working alongside other senior social work staff, the role is responsible for the supervision, mentoring, training, and performance management of Level 1 and Level 2 Social Workers, as well as other staff as directed by the Director of Social Work. The position also supports the ongoing enhancement and effective delivery of social work services within the team.

Services are provided across a variety of community settings, including clients’ homes, outpatient clinics, and group programs. The role requires regular travel throughout the Central Coast region.

About You

You will bring a strong commitment to high-quality community care and a passion for helping people. You will have:

  • Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and hold current driver’s licence (overseas qualifications must supply the AASW Overseas Assessment number).
  • Demonstrated application of extensive clinical experience across a diverse and broad range of diagnoses and conditions within the community setting (including identified skills detailed in the advertisement and task list).
  • Extensive skills and experience in clinical/professional supervision of Social Work staff and students, and demonstrated experience in effectively managing variations in staff performance and expertise.
  • Demonstrated skills and team management and evaluation, both within social work teams and multidisciplinary teams, and demonstrated understanding of the current AASW Practice Standards as they relate to the position.
  • Demonstrated high level of broad and diverse assessment, intervention and group work skills and demonstrated commitment to the ongoing maintenance, development and currency of professional skills and knowledge.
