At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are looking for an experienced Occupational Therapist Level 3 to join our Transitional Aged Care Program (TACP) team.


About the Opportunity

As a Senior Occupational Therapist, you will play a key role within our multidisciplinary allied health team, delivering high-quality, evidence-based care to older people transitioning from hospital back to the community. Transitional Aged Care is a goal-oriented, time-limited program designed to optimise independence, improve function, and support older people to remain living at home for longer.

This position provides clinical leadership across community and residential Transitional Aged Care services and contributes to the ongoing development of occupational therapy practice within the program. In this role you will:

  • Deliver advanced Occupational Therapy assessment and intervention services to older people with complex health needs, supporting their functional goals and independence.
  • Provide clinical leadership, supervision, and mentoring to Occupational Therapists, Allied Health Assistants, and students.
  • Collaborate with clients, families, carers, and multidisciplinary team members to ensure coordinated, person-centred care and seamless transitions from hospital to community.
  • Lead and contribute to quality improvement, service development, and evidence-based practice initiatives that enhance patient outcomes and service delivery.
  • Manage a complex caseload independently while exercising advanced clinical judgement and problem-solving skills.

About You

We are looking for someone who has:

  • Appropriate qualifications as an Occupational Therapist and current general registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Extensive clinical experience delivering Occupational Therapy services to older adults with chronic and complex health conditions, including neurological, stroke, cardiac, and respiratory presentations.
  • Demonstrated experience working within community-based services, including home assessments, equipment prescription, and home modification recommendations.
  • Strong leadership, supervision, communication, and organisational skills, with the ability to effectively manage competing priorities.
  • A collaborative approach and commitment to working within a multidisciplinary team to deliver high-quality, person-centred care.
  • A current unrestricted NSW Driver Licence and willingness to travel across sites as required.
