About the Opportunity
Join a dedicated multidisciplinary team supporting young people aged 12–25 years to access the right mental health, wellbeing, and support services. As a Youth Access Team (YAT) Worker, you will play a pivotal role in engaging young people and their families, providing assessment, intervention, and referral pathways that promote positive outcomes.
In this role you will:
- Conduct bio-psychosocial assessments and provide brief interventions for young people experiencing mental health, alcohol and other drug, or psychosocial challenges.
- Triage and coordinate referrals to appropriate internal and external health, mental health, and community support services.
- Build trusted relationships with young people, families, carers, and key stakeholders to support person-centred care.
- Collaborate with multidisciplinary teams, GPs, allied health professionals, and community agencies to ensure seamless service delivery.
- Advocate for young people and promote youth-friendly, recovery-oriented approaches to care.
- Maintain accurate clinical documentation, data collection, and contribute to ongoing service improvement initiatives.

- Occupational Therapist Level 1 and 2
- Social Worker Level 1 and 2
- Welfare Officer Level 1 and 2
- Psychologist
- Registered Nurse

About You
We are looking for someone who has:
- Experience working with young people and a strong understanding of the mental health, wellbeing, and support needs of people aged 12–25 years.
- Well-developed communication and engagement skills, with the ability to build effective relationships with young people, families, carers, and stakeholders.
- Experience conducting assessments, providing brief interventions, and coordinating referrals within a health, mental health, or community services setting.
- The ability to work autonomously while contributing positively within a multidisciplinary team environment.
- Strong organisational, documentation, and stakeholder management skills, with a commitment to delivering youth-friendly, person-centred care.
