Central Coast Local Health District is seeking a committed and culturally capable professional to join our Child and Adolescent Mental Health Services (CAMHS) team as a Service Navigator. This newly developed role is designed to provide targeted support to Aboriginal and Torres Strait Islander children, young people, and their families/carers who present with suspected co-morbid neurodevelopmental, behavioural, and mental health concerns.

The position is responsible for assisting individuals and families in accessing appropriate health and community-based services. It plays a key role in establishing and strengthening collaborative pathways between CCLHD, Non-Government Organisations (NGOs), and Community Managed Organisations (CMOs), enhancing coordinated care and improving service access for this priority population.

This role is graded for the following:

  • Clinical Nurse/Midwife Specialist Grade 2
  • Dietitian Level 3
  • Occupational Therapist Level 3
  • Physiotherapist Level 3
  • Social Worker Level 3
  • Speech Pathologist Level 3
  • Clinical Psychologist
  • Senior Psychologist
  • Counsellor Level 3

Key Responsibilities

  • Provide culturally responsive service navigation support to Aboriginal and Torres Strait Islander clients within the CAMHS framework.

  • Facilitate timely and appropriate referrals to internal and external services.

  • Build and maintain strong partnerships with health, community, and non-government stakeholders.

  • Support the integration of care across service sectors, with a focus on early intervention and culturally safe practices.

  • Advocate for the inclusion of Aboriginal perspectives in mental health service planning and delivery.

About You: 

  • Demonstrated experience working with Aboriginal children, young people, and their families, preferably within health, mental health, or community settings.
  • Knowledge of local service systems and ability to navigate health and community pathways effectively.
  • Proven ability to build trusting relationships and work collaboratively with diverse stakeholders.
  • You are a passionate advocate for inter and multidisciplinary care planning, serving as a crucial information connector within the Central Coast Local Health District (CCLHD).  Your role bridges the gap between CCLHD staff, Chief Medical Officers (CMOs), and the private sector, fostering strong relationships and ensuring seamless communication.
