Transition Support Worker - Targeted

We are looking for someone who can: 

·       Support young people and their families through the healthcare transition process by managing enquiries, providing information, and assisting with referrals and appointments.

·       Maintain accurate records of all communications and actions, including data entry, discharge processing, and follow-up post-transfer.

·       Work collaboratively with the Transition Care Coordinator to ensure smooth service delivery, clear communication, and up-to-date resources for stakeholders.

What you will be doing  

The primary purpose of the Transition Support Worker role is to provide support for the Transition Care Coordinator (TCC) to help young people with chronic illness/disability transitioning from paediatric to adult health services throughout NSW and to improve process across the health system.

The Transition Support Worker is a key member of the Transition Care Service team. This is a part-time role, working two days per week, with designated anchor office days based in St Leonards.

Remuneration: $68,338.79 - $70,468.72 pro rata per annum excluding Superannuation

Selection Criteria

1. Experience working with young people with chronic illnesses and disabilities.

2. Ability to work as an effective member of a team with the capacity to undertake a diverse range of tasks and also to work with minimal direction when required.

3. Strong communication skills including experience of communicating with young people.
